Enrollment Requirements

Enrollments occur twice a year: February-May and August-November. At enrollment FSA paperwork is verified, NRCS practice is chosen for implementation, and farm data is collected for fields enrolled. The enrollment questionnaire allows Elevated Foods to determine if you are a good fit for the AMP program.

Subsidiary Prints are generated by USDA Farm Service Agency (FSA) to document eligibility criteria for various USDA programs.

The submitted Subsidiary Print must show AD-1026 certification, HELC compliance, and Converted Wetlands compliance for the enrollment year.

The 156EZ is a form used by the FSA to establish and maintain farm records for farmers seeking to participate in USDA programs. The form collects essential information about the farm, including operator details, land data, and associated farm numbers.

The submitted form must be for the enrollment crop year and show each farm and tract as available cropland.

Maps for each FSA registered farm, tract, and field are required. They must be colorized, from the year of application, and show fields marked as 'cropland' to be eligible.

An EE (Environmental Evaluation) must be completed and a form CPA-52 submitted for any land enrolled in the program.

A practice must be chosen to be implemented for each FSA field during the contract period. Eligible practices are:

  • Reduced-Till/ No-Till (NRCS 329 & 345) 
  • Nutrient Management (NRCS 590) 
  • Cover Cropping (NRCS 340) 
  • Soil Amendment (NRCS 336) 
  • Field Borders (NRCS 386)- Requires additional verification with the USDA

A workbook that captures enrollment data about the FSA registered farm, tract, field, and crops enrolled in the program. This is updated during reporting periods if there are crop changes. Initial enrollment workbook will be completed with a member of EF's staff.

An updated W-9 form ensures Elevated Foods has the correct taxpayer information for proper tax reporting to the IRS.

Reporting Requirements

Reporting for the grant is completed every six months and entails reporting on crop, farm, and sales data. At the end of the contract year, implementation and soil sample receipts are also collected. Required Reporting Documents:

Similar to the enrollment workbook, but with questions around practice implementation, sales data for enrolled fields, and confirmation of crops. This will be completed with a member of EF's staff.

Receipts showing the reported cost to implement the NRCS practice. This can include costs of labor, materials, or equipment.

Receipts and results for soil samples at enrollment and, if in your final contract year, at the end of your contract.
